Independent Patient Safety Incident Reviews
We provide expert independent systems analysis reviews of patient safety incidents. Our incident review process is guided by human factors approaches and, underpinned by restorative just culture to ensure blame is avoided and system issues are identified and improved.
Clinical Practice Guideline Review
Whether your organisation uses clinical practice guidelines (CPG), protocols, or medical directives, our team will work with you to review CPG structures using “Work-as-done” versus “Work-as-imagined” and Failure Modes and Effects (FMEA) framework to analyse proactively safety system issues before they are realised.

Error analysis
Our experts can analyse incident data to thematically identify incident trends. These are sometimes referred to as “sentinel events” and are often a collection of less severe or near miss (good catch) incidents that serve as precursors to larger events. Safety culture is often talked about, but analysing, growing and embedding it as part of day to day operations is never as simple as it seems. Education and Training
We believe that empowering paramedics, paramedic services, and regulators to become leaders in patient & paramedic safety is the only way forward – building capacity within the profession. While in some circumstances our training can be delivered remotely, we strongly encourage face to face engagement and facilitation. While training is designed for paramedicine, it can be tailored to community healthcare organisations. All modules are informed by evidence based research and in some cases a train the trainer session can be added to a specific course.
Intro to human factors in paramedicine
This training module will introduce human factors as a scientific discipline and will connect core concepts, theories and principles to paramedicine. Topics include: Incident and Error, Safety 1 and Safety 2, What Human Factors Is (and Isn’t), Analysing Systems, and more.

Conducting systems analysis
This comprehensive training module will take practitioners through a HF/E informed patient safety review. Utilising frameworks such as SEIPS, The London Protocol, Thematic Analysis, etc…, participants will learn how to conduct a patient safety review from start to finish, develop recommendations and draft comprehensive systems analysis reports.

Teamstepps and CRM
TeamSTEPPS encompasses Crew Resource Management (CRM) and aims to improve team performance across healthcare. In this module we apply TeamSTEPPS principles to paramedicine to improve situational awareness, communication, leadership, team structure and mutual support.

After Action Reviews
After Action Reviews (AAR) are useful both as a formal review tool and informal debrief methodology to examine clinical events. This training module will introduce practitioners to AAR both as a formal review tool and as an informal debrief and learning tool.

Safety 1, Safety 2 and LFE
This module builds on Introduction to Human Factors and explores Safety 1, Safety 2 and Learning From Everything (LFE). Safety 1 is generally aligned with learning from incidents – “when things go wrong”. Whereas Safety 2 focuses on learning from “when things go right”. Finally the case is made that organisations should LFE to improve safety and performance.

Just Culture
Restorative Just Culture is a major component of any safety system and fundamental to safety culture. In this module participants will learn what Just Culture is (and isn’t), explore Restorative Just Culture – a new emerging model. Participants will learn how to embed and enact just culture within their organisations, underpinned by psychological safety.
